We welcome all patients new to the area who wish to register with this practice. We have a practice boundary which is displayed in reception and can only register patients living within this boundary.
If you decide to register with the practice we ask that you complete a registration form and questionnaire. It is not essential but we would advice that you make an appointment with our healthcare assistant who will ask you a few questions regarding your previous medical history. This is because it may be some time before we receive your medical records from your previous GP and we will have some medical information should you wish to see a doctor or nurse prior to your medical records reaching us.
If we cannot accept you onto the list you will be given a full written explanation outlining the reason/s for this. New patients are registered with the practice and not a specific doctor. However, at the time of registration you have the right to ask to be registered with a specific doctor and we will make every effort to see that you see this doctor whenever possible. Due to holidays or sickness this may not always be possible but we will always offer an alternative if you wish to be seen urgently.
Alternatively, you can wait until an appointment is available for the doctor or your choice. New patients are welcome to visit the surgery and meet any of the doctors and staff before joining the list. All newly registered patients over the age of five years will be offered an appointment for a registration medical with our practice nurse.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
